You can even email us your forms prior to your appointment to speed up your first visit. Or if you’d prefer to complete them in our office, please plan to arrive 15 minutes prior to your appointment time.

Let us save you the hassle of dealing with insurance companies! Simply email a copy of the front and back of your insurance card, as well as your date of birth, to amie@ihsindy.com and we’ll verify your coverage for you.
